Insurance
Stacey Mason, LPC, LCADC is an out-of-network provider and does not accept insurance, so payment is due prior to the time service is rendered. A statement that you can submit directly to your insurance carrier will be provided. Please check with your insurance to see if you are covered and what the reimbursement would be, if any. If you have health insurance, and the services you are seeking are covered by your health care plan, you may be able to get the items or services described in this notice from providers who are in-network with your health plan.
Some questions to ask your health insurance company:
- Do I have mental health insurance benefits for my family?
- What is the reimbursement percentage for an out of network provider?
- How many sessions per year will my insurance cover?
- Do I need to meet a deductible? If so, how much?
Rates: $175 per session
CONFIDENTIALITY/PRIVACY
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission. There are three exceptions:
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s. I must notify the police and inform the intended victim.
- If a client intends to harm him/herself. I will make every effort to enlist their cooperation in insuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
